Policies


Our staff commits to:

1.Host you in our office with the dignity afforded to a guest

2.Care for you with respect

3.Trust that you know your own body better than anyone else does

4.Explain your condition, possible treatments, and known risks to you in plain English

5.Empower you to make decisions regarding your health care

6.Listen to you and do our best to answer any questions you may have about your health

7.Give you the opportunity to agree to or refuse any treatment, procedure or test before it begins

8.Respect the confidentiality of all your medical care and records

9.Provide you with descriptions of our policies (below) with which we ask you to abide


As a patient, you promise to:

1.Supply complete and accurate medical history information

2.Ask the staff to explain any information or instructions that you do not understand

3.Actively participate in your own health care

4.Follow the treatment plans

5.Inform the staff if you feel that the treatment plan does not meet your needs or is too difficult to follow

6.Give the office at least 24 business hours notice if you need to reschedule an appointment

7.Pay when service is provided


POLICIES:

Appointments - We respect your time and we do our best to stay on schedule.  Please arrive on time for your appointments so that you will have your full allotted time with the doctor.  Please be advised that you are responsible to call 24 business hours before a scheduled appointment if you need to reschedule. 

Payment - Payment is required at time of service.
We accept: Cash, Check, Visa, MasterCard, American Express, Discover

Insurance - Many of the services offer by Dr. Kennedy are covered by health insurance companies.  However, Dr. Kennedy does not submit or accept payments directly from insurance companies.  We help our patients to get reimbursement from their insurance company by providing a coded receipt upon request.  If you want to find out what your plan will cover, call your insurance company and ask "what percentage of the bill will you cover if I go to see an "out-of-network" physician?

Telephone Calls - The doctor accepts EMERGENCY calls from established patients any time, day or night.  If you have a life-threatening emergency, please call 911 first, then call our office. If you have a routine medical question, please call us any day between 9:00 AM and 5:00 PM.  For administrative questions, please call Monday - Friday from 9 AM-5 PM. If you reach our voicemail, you may leave a confidential message and the doctor or staff will get back to you as soon as possible.  Calls of less than 10 minutes from/to established patients regarding current treatment are free.  Calls about new issues will be billed at our regular rate.
